Requests for Release of Provider Transaction Access Number (PTAN) (GEN)

The Centers for Medicare & Medicaid Services (CMS) Division of Provider and Supplier Enrollment provided the following guidance regarding inquiries requesting Practitioner/Provider Transaction Access Numbers (PTANs).
Medicare contractors shall not provide names and/or legacy identifiers of any provider or supplier, including physicians, to any individual or medical organization. CMS believes that legitimate medical organizations who have maintained and updated their enrollment information with Medicare in a timely manner would know the names of the individuals (and their legacy identifiers) that are associated with the medical organization.
If an individual provider requests their PTAN, it can be mailed to their practice location. The contractor should never give information over the phone. If an organization or group needs to know the PTAN of a provider, then they should contact the provider(s) and have them request their own PTAN from the contractor. Once the contractor receives the request from the provider, the PTAN will be mailed to the provider's practice location.
Therefore, effective immediately, all requests for PTANs must be made in writing and directed to Upstate Medicare Division, General Correspondence, P.O. Box 5302, Binghamton, NY 13902-5302.
All requests for PTANs must be made by the provider. Requests for a group PTAN must be made by the authorized or delegated official on file. These requests must be submitted in writing on the provider's/entity's letterhead and mailed to the address noted above. The PTAN will be mailed to the attention of the provider at the primary practice location for that PTAN/provider.
If the PTAN was issued less than 10 days prior to the request, the provider will be advised that the PTAN was mailed to the pay-to-address on file, unless the application requested otherwise. |
