New Electronic Funds Transfer (EFT) Authorization Agreement (CMS-588) (GEN)

The Centers for Medicare & Medicaid Services (CMS) has issued revisions to the CMS-588 Electronic Funds Transfer (EFT) Authorization Agreement. The revisions include a section for a contact person, telephone number, fax number, email address, and mailing address for EFT verification.

Contractors shall continue to accept either the 9/2003 or the 9/2006 version of the EFT Agreement through November 30, 2006.

Effective December 1, 2006, contractors shall only accept the 9/2006 version of the EFT Authorization Agreement. If the 9/2003 version is submitted, it will not be accepted.

The new EFT Authorization Agreement can be downloaded from the CMS Web site at www.cms.hhs.gov/cmsforms/downloads/cms588.pdf.
