National Plan and Provider Enumeration System (NPPES) Data Dissemination Notice (CMS-6060-N) (GEN)

The Centers for Medicare & Medicaid Services (CMS) published the National Plan and Provider Enumeration System (NPPES) Data Dissemination Notice in the Federal Register. The Notice is now on display at the Office of the Federal Register. It was published on May 30, 2007.

The Notice encourages health care providers who have been assigned National Provider Identifiers (NPIs) to review their NPPES data at this time and make any necessary updates or corrections to ensure that their information is accurate when disclosed by CMS. (Health care providers who are covered entities under the Health Insurance Portability and Accountability Act (HIPAA) are required by regulation to update their NPPES data within 30 days of any change.)

Additional Information
In order to enroll a provider and issue a Provider Transaction Access Number (PTAN), the following matches must be made with the NPPES database information:
  • For an individual: NPI - exact, Social Security Number (SSN) - exact, Date of Birth (DOB) - exact, First Name - first initial, Last Name - first seven letters.


  • For an organization: NPI - exact, Employer Identification Number (EIN) - exact, Legal Business Name (IRS-CP575 match) - exact.
If the information a provider is submitting to Medicare does not match what is on file with NPPES, the application cannot be processed.

More information and education on the NPI can be found at the CMS NPI page at www.cms.hhs.gov/NationalProvIdentStand on the CMS Web site. Providers can apply for an NPI online at https://nppes.cms.hhs.gov or can call the NPI Enumerator to request a paper application at 800-465-3203.

Getting an NPI is free - not having one can be costly.
