Triesence™ (Triamcinalone Acetonide Injectable Suspension) (GEN) - Revised 7-21-08

This drug was Food and Drug Administration (FDA) approved for intraocular injection on November 29, 2007. Ophthalmologists should determine use as advised in the FDA approved package label, since there are multiple indications. Please use HCPCS code J3490 (Unclassified drug), or if you are billing for an ambulatory surgical center (ASC), use code C9399 (Unclassified drug or biological). The drug name should be identified in the narrative section of the electronic claim or in Item 19 on paper claims.

Triesence™ comes in a single use vial and will be paid in a quantity billed (QB) of 1. If it is injected in both eyes, the QB should be 2, since a new vial is to be used for each eye. A drug invoice will be requested until the Upstate Medicare Division (UMD) can establish appropriate pricing.

Please note: Previously, there was intraocular injection of triamcinolone acetonide, Kenalog 10 mg., J3301, which was off label and not FDA approved. Kenalog is now specifically labeled, not for intraocular use.
