Posted March 20, 2008
On page 23 of the June 2005 Medicare B Hotline Bulletin, we published MLN Matters article SE0529, titled, “Importance of Supplying Correct Provider Identification Information Required in Items 17, 17a, 24K, and 33 of the Form CMS-1500, and the Electronic Equivalent.” This article was revised on March 11, 2008, to clarify that all references to the form should state CMS-1500 (12-90). Providers may also want to refer to MLN Matters article MM5060 at www.cms.hhs.gov/MLNMattersArticles/downloads/MM5060.pdf, which states the requirements for the newer form, CMS-1500 (08-05). The previous revision to the article added a reference to MLN Matters MM5890 (www.cms.hhs.gov/MLNMattersArticles/downloads/MM5890.pdf). MM5890 stated that effective with claims received on or after May 23, 2008, Medicare will not pay for referred or ordered services or items, unless the fields for the name and NPI of the ordering, referring and attending, operating, other, or service facility providers are completed on the claims.
