Ixempra™ (Ixabepilone) (GEN) - Revised 4-9-08

Effective for dates of service on or after October 16, 2007.

Ixempra™, a microtubule inhibitor, in combination with capecitabine is indicated for the treatment of metastatic or locally advanced breast cancer in patients after failure of an anthracycline and a taxane.

Ixempra™ as monotherapy is indicated for the treatment of metastatic or locally advanced breast cancer in patients after failure of an anthracycline, a taxane, and capecitabine.

The drug should be billed using HCPCS code J9999 (for dates of service before April 1, 2008) or HCPCS code Q4082 (for dates of service on or after April 1, 2008) with the appropriate diagnosis code for the patient's condition. The name of the drug and the dosage administered should be entered in the narrative field, along with a brief statement about how the patient meets the above criteria, i.e., resistant to anthracyclines, taxanes, and capecitabine.
