You may use any low risk diagnosis that is applicable to the patient. The Medical Director's office cannot designate a specific diagnosis to use with code G0121 because the choices available are too varied. Please refer to Pub. 100-02, Medicare Benefit Policy Manual, Chapter 15, §280.2 for information regarding coverage and billing requirements for colorectal cancer screening tests.
